Low health risk

(including Diabetes Mellitus, Hypertension, Low back pain, Degenerative knee pain)

  • Refer to medical M&H practitioner for screening and diagnosis (Diabetes Mellitus, Hypertension)
  • Follow-up services for Low back pain, Degenerative knee pain (Referred by Network M&H Practitioners)
  • Participate in Chronic Disease Management Programmes, including
    • Patient Empowerment Programme (PEP)
    • Individual healthcare services (If needed) (subsidised by the Government)
    • Free Consultation(s) with a Nurse, Pharmacist, Nutritionist, Physical Therapist, Occupational Therapist and Social Worker

  • Health seminars and class activities
  • Access to smart gym facilities in the Centre
